Personal Data Protection Disclosure Statement

 This disclosure statement has been prepared by Berrin Pehlivan, who provides services at Krizantem Sok. No:30 34330 Levent/Istanbul with Tax ID Number 7240156312 (hereinafter referred to as “ONKOLOG”), in accordance with Articles 10 and 13 of the Personal Data Protection Law No. 6698 (“Law”) and the Communiqué on Principles and Procedures to be Followed for Fulfillment of Disclosure Obligation. ONKOLOG demonstrates utmost sensitivity regarding the confidentiality and security of personal data. This disclosure statement is prepared within the scope of the relevant law.

What is Personal Data?

 Personal data is defined in the Law as any information related to an identified or identifiable real person. Accordingly, personal data that you share with us, including; your name, surname, date of birth, identification number under the identity category; your email address, phone number, address under the contact category; and personal health data, such as height, weight, and chronic illnesses, are considered special categories of personal data.

What Are the Purposes and Methods of Processing Your Personal Data?

ONKOLOG collects and processes your personal data in compliance with the data processing conditions stipulated in the legislation to fulfill our legal obligations, support regarding record keeping, communication, healthy living, lifestyle, and to inform you about campaigns and advantages.

 In this regard, your personal data may be stored, processed, and transferred for the following purposes:

  • Confirming your identity,
  • Preparing and implementing appropriate treatment and healthy living programs, • Sharing requested information with public institutions and organizations in accordance with relevant legislation,
  • Planning and managing the internal operations and daily activities of the Consulting Center and its affiliated or associated centers,
  • Measuring client satisfaction, contacting you regarding our services,
  • Conducting customer acquisition activities and sharing offers related to our services,
  • Notifying you about appointments if you make one,
  • Performing risk management and quality improvement activities,
  • Meeting legal and regulatory requirements,
  • Participation in campaigns and receiving campaign information by marketing, media, communication, and call center authorities,
  • Designing and delivering special content and benefits on web and mobile channels.

Additionally, your personal data may be processed automatically or manually without the data subject’s consent under the following conditions as per Article 5(2) of the Law No. 6698:

  • Explicitly provided for by laws,
  • Necessary for the protection of life or physical integrity of a person who is unable to give consent due to physical impossibility or whose consent is not legally valid,
  • Necessary for the establishment or performance of a contract with the data subject,
  • Mandatory for the data controller to fulfill a legal obligation,
  • Made public by the data subject,
  • Necessary for the establishment, use, or protection of a right,
  • Necessary for the legitimate interests of the data controller, provided that it does not harm the data subject’s fundamental rights and freedoms.

 For What Purposes and to Whom Do We Transfer Your Personal Data?

 Your personal data may be shared with domestic and international individuals and institutions (lawyers, auditors, tax consultants, other third parties we consult with and provide services from, courier, mail, call center, database, cloud, company employees, etc.) who collaborate with us for legal and financial follow-up and execution of business activities. Additionally, personal data may be shared with courts and other public institutions within the framework of mutually agreed confidentiality policies due to legal obligations.

What Are Your Rights Regarding the Protection of Your Personal Data?

Under the Regulation, you have the right to access your personal data, request the correction, deletion, or restriction of processing of your personal data, object to the processing activities, file a complaint with a supervisory authority, receive your personal data in a structured, commonly used, and machine-readable format, and transfer this data to another data controller, if technically feasible (data portability).

According to Article 11 of the Law, the data subject has the rights to: (i) learn whether their personal data is being processed, (ii) request information if their personal data has been processed, (iii) learn the purpose of processing personal data and whether it is used for its intended purpose, (iv) know the third parties to whom personal data is transferred, (v) request the correction of incomplete or incorrect personal data and to have this correction notified to third parties to whom personal data has been transferred, (vi) request the deletion or destruction of personal data when the reasons for processing no longer exist, and to have this request notified to third parties to whom personal data has been transferred, (vii) object to the emergence of a result against them by means of exclusively automated processing, and (viii) request compensation for damages arising from the unlawful processing of personal data.

Requests related to the exercise of these rights can be submitted to us using the methods specified in the Personal Data Processing and Protection Policy within the scope of Law No. 6698. ONKOLOG will evaluate these requests and conclude them as soon as possible and within a maximum of 30 days. ONKOLOG reserves the right to charge fees based on the tariff determined by the Personal Data Protection Board (if applicable).
